新型冠狀病毒肺炎體溫回報表－個人資料蒐集聲明 (COVID-19 Prevention Related – Protection of Personal Data Announcement)

鑒於「新型冠狀病毒肺炎」疫情擴大且國內出現人傳人案例，參酌我國疾病管制署之防疫建議，並基於保護采鈺科技股份有限公司（下稱「本公司」）全體員工、訪客、來賓及廠商的身體生命安全，進入本公司均須配合填寫體溫回報表。本公司透過體溫回報表所蒐集、處理及利用之廠商個人資料類別包含您的識別類個人資料（姓名）、身體狀況、聯絡方式、社會活動及其他為防疫所需之個人資料，除上述之防疫目的（下稱「蒐集目的」）外不另作其他用途。

In light of the fact that COVID-19 cases continue to grow and the Taiwan Centers for Disease Control (Taiwan CDC) has confirmed the domestic human-to-human transmission case. In accordance with Taiwan CDC’s suggestion and in order to protect the health of all the employees, visitors, and vendors of the Taiwan Semiconductor Manufacturing Company (“VisEra”), all the people who are entering VisEra areas must cooperate to fill in the “Self-check Body Temperature Form”. Your personal data collected by VisEra via this “Self-check Body Temperature Form” contains your personal identification information (i.e., your name), physical condition, contact information, data concerning your social activities, and other data that is necessary for us to take anti-epidemic measures. Your personal data will not be used for any purpose other than the aforementioned epidemic prevention purposes (“Collection Purpose”).

您的個人資料，僅供本公司於台灣境內設有辦公場所或廠房之地區使用，於蒐集目的之必要合理範圍內加以利用至蒐集目的消失時為止。

Your personal data will only be used within the areas in Taiwan where VisEra has offices or fabs, and within the necessary and reasonable scope for the Collection Purpose until the Collection Purpose is no longer applicable.

依據個人資料保護法第3條規定，您可向本公司請求查詢或閱覽、製給複製本、補充或更正、停止蒐集/處理/利用或刪除您的個人資料。
